[BCOR::CCNB3 fusion sarcoma: a clinicopathological analysis of three cases]

Abstract

目的: 探讨BCOR::CCNB3基因融合肉瘤(BCOR-CCNB3 fusion sarcoma,BCS)临床病理特征、诊断及鉴别诊断要点。 方法: 收集南京医科大学第一附属医院2020—2022年确诊的3例BCS,镜下观察HE切片及免疫组织化学染色结果,并应用荧光原位杂交(FISH)法检测其分子遗传学改变,复习相关文献进行分析。 结果: 3例患者均为男性,肿瘤分别位于右侧竖脊肌旁、右大腿、第5腰椎椎体。镜下肿瘤细胞小至中等大,呈圆形、短梭形,核染色质细腻,核分裂象易见,排列呈实性片状、束状、局灶旋涡状,间质可见复杂分支血管网,部分区域间质呈疏松黏液样,其内细胞呈梭形,排列成细条索状。免疫组织化学显示3例肿瘤细胞均表达BCOR、cyclin D1、SATB2、INI1,部分表达TLE1及CD99。FISH检测显示3例均存在BCOR基因断裂重排和BCOR::CCNB3基因融合。 结论: BCS是一种骨与软组织系统中罕见的小圆细胞未分化肉瘤,好发于年轻男性,需与多种肿瘤相鉴别,熟悉其组织形态学及免疫组织化学特征并辅以特征性的分子检测,可避免误诊。.
