[Study on lateral hinge fracture during open wedge high tibial osteotomy]

Abstract

Objective: To summarize the related research results of open wedge high tibial osteotomy (OWHTO) complicated with lateral hinge fracture.

Methods: To review the relevant literature of OWHTO at home and abroad in recent years and summarize and analyse the clinical experience.

Results: The lateral hinge rupture may occur during the OWHTO, which may lead to the loss of correction angle after operation, delayed healing or non-union of osteotomy and so on. The lateral hinge plays an important role in the stability of the osteotomy. During the operation, the "safe zone" internal osteotomy can be used to protect the bone. Once the lateral hinge breaks, the TomoFix plate can be used to obtain the sufficient stability. For patients with lateral hinge rupture, functional exercise and full weight loading time should be guided by hinge breakage classification.

Conclusion: The intact lateral hinge is beneficial to the healing and rehabilitation of OWHTO. The lateral hinge should be paid enough attention by clinicians.

目的: 总结开放性楔形胫骨高位截骨术(open wedge high tibial osteotomy,OWHTO)术中并发外侧铰链断裂的相关研究成果。.

方法: 查阅近年国内外 OWHTO 相关文献,结合临床经验进行总结分析。.

结果: OWHTO 术中发生外侧铰链断裂后,导致术后矫正角度丢失、截骨延迟愈合或不愈合等问题。外侧铰链对截骨术后稳定性起重要作用,术中可以采用“安全区”内截骨保护措施;当发生外侧铰链断裂时可选择 TomoFix 锁定钢板固定,以提供足够稳定性。对于发生外侧铰链断裂患者,可根据铰链断裂分型指导功能锻炼及确定完全负重时间。.

结论: 完整外侧铰链利于 OWHTO 术后愈合及康复,OWHTO 术中医师应高度重视,避免发生外侧铰链断裂。.
