The term «crazy paving» describes an unspecific pattern in thoracic computed-tomography mimicking a paving stone configuration. This pattern can be caused by a number of differential diagnosis, for example alveolar hemorrhage, alveolar proteinosis, lipoid pneumonia or adenocarcinoma in situ. The knowledge of the correlation between imaging findings, histopathological pattern and clinical presentation prepares the clinician to understand the pathophysiology and to initiate the right therapeutic processes. In the following review, we describe the most common diseases and their radiologic correlation in daily practice.
Der Begriff «Crazy paving» beschreibt ein häufig angetroffenes Muster in der thorakalen Computertomographie, das bildmorphologisch einem Pflastersteinrelief ähnelt. Ätiologisch ist es einer Reihe von Differenzialdiagnosen zuzuordnen, unter anderem der alveolären Hämorrhagie, der alveolären Proteinose, der lipoiden Pneumonie sowie des Adenokarzinoms in situ. Das Wissen um den Zusammenhang zwischen radiologischem Muster, histopathologischem Korrelat und klinischer Repräsentation erleichtert dem Kliniker das adäquate Verständnis des Befundes mit entsprechender therapeutischer Relevanz. Im vorliegenden Review beschreiben wir im Detail den relevanten Kontext der am häufigsten in der radiologischen Routine anzutreffenden Entitäten.
La sémiologie radiologique du «crazy paving» représente un pattern en TDM thoracique imitant un aspect «en pavé» dans les images. Cet aspect peut être provoqué par nombreuses maladies dont les plus importantes sont l'hémorrhagie alvéolaire, la protéinose alvéolaire, la pneumonie lipoide et le carcinome bronchoalvéolaire. La synthèse entre les signes en TDM thoracique, l'anatomopathologie et la présentation clinique est importante pour comprendre le contexte des maladies et initier le traitement approprie. Ci-dessous nous décrivons les maladies les plus importantes et la corrélation avec l'image TDM thoracique.
