Eyes that lack sufficient capsular support for intraocular lens (IOL) placement present a unique surgical challenge. Options for IOL placement include anterior chamber intraocular lenses (ACIOLs), iris-fixated posterior chamber intraocular lenses (PCIOLs), and scleral-fixated intraocular lenses (SFIOLs). Effective use of these fixation techniques in the appropriate clinical setting has been described, without clear superiority of one method.

However, SFIOLs provide a better ocular safety profile than ACIOLs or iris fixated IOLs. Placement of IOL in the sulcus region keeps the IOL closer to the natural anatomic position of the crystalline lens and is safer for the corneal endothelium and iris.
