Pharmacological analysis of patient's charts in a long-term care hospital

Abstract

Pharmacological study of patient's medication record is a pharmaceutical task acknowledged by Quebec Pharmacy Act. Authors present here the sequence of this analysis and the selected criteria used to evaluate the drug utilization in their facility. Results show that 97 patients' medication records did not conform with screening criterion in regard to the number of drugs. Of these records 74 failed to pass the indepth criteria. Deficiencies were identified and evaluated with physicians. Pharmacist's recommendations were accepted and followed by physicians for 53% of the cases.
